Documents necessary for vehicle claims settlement

If you want to add information to an already reported claim, use lpu@slavia-pojistovna.cz; don’t forget to include the claim number in the email subject.

What documents do you need for a particular insurance?
Claim reporting
- Copy of vehicle registration
- Copy of driving licence
- Accident record – duly completed and signed by both parties involved in the accident
From the party at fault
- Claim reporting (by telephone, filling in the online form, download form)
- Copy of vehicle registration
- Copy of driver’s driving licence at the time of the accident
- Traffic accident record – signed by both parties involved in the accident, or confirmation from the police
From the injured party
- Claim reporting (by telephone, filling in the online form, download form)
- Copy of the vehicle registration (if the vehicle is leased, release of funds must also be documented)
- Copy of the driver’s driving licence at the time of the accident (if the vehicle was parked, it is not necessary)
- Request for benefit in the form of a budget (free form) with a bank account number, or an invoice from a repair shop with a power of attorney for reimbursement
- Bank account for payment of benefit or power of attorney for payment to a repair shop
- Copy of the accident record – signed by both parties involved in the accident, or a confirmation from the police
If they use a replacement vehicle
- Replacement vehicle lease agreement
- Justification of need (solemn declaration)
- Handover certificate
- Copy of the vehicle registration
- Replacement vehicle lending invoice
- Proof of replacement vehicle payment
- Bank details for sending benefit for a replacement vehicle
Vehicle accident insurance, windscreen, collision with wild animals
- Copy of the vehicle registration
- Copy of the driver’s driving licence at the time of the accident (if the vehicle was parked, it’s not necessary)
- Invoice from a repair shop with a power of attorney for reimbursement, or proof of invoice payment with a bank account number
- Confirmation from the police – collision with wild animals only
- Conclusive photo documentation from a repair shop – windscreen only
Necessary photo documentation of the windscreen
If the windscreen only is damaged, the photo documentation is provided by the repair shop
- Damage details
- Removed damaged glass leaning against the vehicle, with visible vehicle plate number
- Vehicle with new glass with visible vehicle plate number
- VIN code, tachometer
- Vehicle from the front and rear with visible vehicle plate numbers
